Job overview
The Host Home Provider role is based in Northern Colorado and requires the provider to live in Loveland, Fort Collins, Greeley, Longmont, Windsor, or Wellington, CO. Documentation and regular communication with ATTUF Group staff and the individual’s support network help coordinate consistent care.
What you'll do
- Provide supervision and daily support based on the individual’s needs
- assist with activities of daily living
- support Individualized Service Plan goals, independence, life skills, and community involvement
- assist with transportation
- administer or assist with medications as delegated and documented
- monitor and report health or behavior changes
- maintain a safe, clean, accessible home
- maintain service, medication, and incident documentation
- participate in team meetings and communicate with staff, case managers, guardians, and service coordinators.
