Physical Therapist Home Care (PRN)

Provides evaluation and physical therapy treatment in accordance with agency standards and state laws. Analyzes and documents patient response to interventions, revises plans of care, and makes recommendations. Establishes and documents teaching plans for patients/families. Contributes to department goals and adheres to quality and safety standards.
