Community Care Coordinator — Utah County

The Community Care Coordinator role is onsite in Utah County, Utah, with an hourly rate of $23. The position is not eligible for remote or hybrid work. NovumHealth’s coordinators help members with behavioral health and substance use needs navigate healthcare systems and stay connected to treatment. What you'll do

• Assess community members’ healthcare needs and identify care gaps
• collaborate on comprehensive care plans
• help patients access appointments, referrals, tests, and community resources
• coordinate appointments across therapy, psychiatry, medication-assisted treatment, and recovery programs
• monitor treatment adherence and provide motivational support
• conduct home or community visits
• document interactions and progress
• participate in case conferences
• transport members as necessary using a personal vehicle, with mileage reimbursed.
