The Registered Dietitian (RD), PACE role is onsite in Grand Junction, CO, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. Pay starts at $32.54–$36.00 per hour, depending on experience. HopeWest PACE supports older adults in living safely and independently in their communities through coordinated, person-centered care. The dietitian assesses participants’ nutritional needs, develops individualized care plans, provides counseling and education, and collaborates with the interdisciplinary team. The role helps maintain participants’ health, independence, and quality of life through nutrition services tailored to their needs and preferences.

What you'll do

• Assess PACE participants’ nutritional status and needs
• develop, implement, and monitor individualized nutrition care plans
• provide counseling and education to participants, families, and caregivers
• collaborate with interdisciplinary team members
• monitor changes and recommend interventions
• participate in care planning and team meetings
• maintain accurate documentation
• help align meals and nutrition services with participant needs and prescribed diets.
