This role exists to provide personalized, one-on-one skilled nursing care to medically complex pediatric and adult patients in community-based settings. The nurse will collaborate with interdisciplinary teams to improve patient quality of life and promote independence.

What you'll do

• Provide one-on-one skilled nursing care in community-based settings.
• Perform ongoing nursing assessments appropriate to your scope of practice.
• Implement and follow individualized Plans of Care.
• Administer medications and treatments as ordered.
• Monitor patient status and report changes in condition.
• Collaborate with physicians, Registered Nurses, and interdisciplinary healthcare professionals.
