ACT Case Manager – Adult Mental Health

What you'll do

• Provide assessments and treatment for adults with severe and persistent mental illness and families
• deliver rehabilitation skills training and case management under the ACT model and applicable state guidelines
• provide advocacy, crisis intervention, individual and group psychosocial rehabilitation, skills training, and other evidence-based treatment allowed by guidelines
• practice trauma-informed and recovery-oriented care
• protect confidential information.

Why this role

The posting requires a bachelor’s degree in social work, sociology, psychology, or counseling and two years of human services experience. A second language relevant to the population served is preferred.
