Residential Care Manager

What you'll do

• Engage members and support persons
• conduct outreach, education, assessment, and care management
• develop and implement care plans
• coordinate healthcare and social services
• advocate for members
• use motivational interviewing, trauma-informed care, and harm reduction
• coordinate discharge planning and referrals
• monitor treatment adherence
• support facility operations and safety
• oversee ECM assessment and care planning
• respond to crises using appropriate procedures.
