The professional provides individualized, person-centered support that promotes choice, dignity, health, safety, daily living skills, and community participation. The schedule is seven consecutive days on followed by seven days off, with day shifts from 7:00 a.m. to 7:00 p.m. open to new hires; picking up additional shifts is optional when available.

What you'll do

• Provide support based on individual needs, preferences, goals, and care plans
• assist with personal care and daily living activities
• support healthcare and medical routines according to instructions
• encourage independent living skills, decision-making, and self-advocacy
• facilitate community inclusion and activities
• monitor well-being and report concerns
• complete documentation and records
• maintain a safe living environment
• follow policies, training, and regulations
• operate company vehicles and transport individuals as needed
• advocate for dignity, independence, inclusion, and quality of life.
