What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, document, and revise plans of care
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• coordinate service frequency with patient acuity and needs
• perform nursing procedures and infection-control practices
• direct hospice aides and LPN/LVNs
• maintain clinical and progress notes
• communicate changes and outcomes to physicians and care personnel
• monitor cases for third-party payer compliance
• provide on-call service as assigned.
