What you'll do

• Provide supervision and support in residential, community, or recuperative care settings
• assist with activities of daily living, including hygiene, dressing, mobility, meals, medication reminders, and toileting
• monitor well-being and report concerns
• support transitions from healthcare, rehabilitation, behavioral health, or correctional settings
• encourage appointments, community activities, and independent living tasks
• maintain daily documentation and progress notes
• follow safety, confidentiality, and agency policies.
