What you'll do

• Complete initial and ongoing case management assessments
• evaluate client needs
• assist with appointments, benefits paperwork, life skills, job searches, and finances
• coordinate discharge, travel, outpatient services, and aftercare placements
• connect clients with community resources
• maintain confidential documentation
• advocate for clients and collaborate with the interdisciplinary team.
