Deliver and document care for Pulmonary Rehab Phase II and III outpatients. Assess patient needs and diagnoses, plan and implement interventions, and evaluate outcomes. Direct and coordinate care based on established Pulmonary Rehab practice standards. Collaborate with other professional disciplines.
