What you'll do

• Evaluate patients and establish and continually assess treatment programs, plans of care, goals, and discharge plans in collaboration with physicians
• implement treatment plans and select activities to progress patients toward goals
• revise goals and plans with patient and family input
• provide patient and family education
• provide discharge instructions, follow-up, and community referrals as appropriate
• document evaluations, plans, treatment sessions, and required education and discharge information.
