What you'll do

• Coordinate, complete, and transmit MDS assessments
• ensure timely and accurate assessments for sub-acute, Medicare, managed care, and long-term care residents
• use PointClickCare for documentation and assessment management
• collaborate with interdisciplinary teams
• conduct resident reviews and chart audits
• lead or participate in the Triple Check Process
• develop and oversee individualized care plans
• monitor assessment schedules
• stay current on regulations and guidelines
• assist with survey preparation and compliance reviews.
