Engagement Care Coordinator

Job overview

The Engagement Care Coordinator role is based in Fairfield County, OH, with in-person and virtual meetings. The pay range is $20.19–$25.03 per hour, based on experience, education, and/or licensure. Integrated Services for Behavioral Health serves Southeastern and Central Ohio through collaborative behavioral health and community services. The coordinator supports youth hospitalized for behavioral health conditions who are enrolled in OhioRISE, helping families navigate discharge, connect with follow-up services, and complete engagement milestones until the youth transitions to an ongoing care coordinator.

What you'll do

• Identify care coordination needs with families
• coordinate family-based services in homes, schools, and communities
• support appropriate care planning and safety needs
• build relationships with youth, families, child and family teams, and community resources
• complete assessments, care plans, crisis safety plans, referrals, and team meetings
• maintain documentation and participate in program evaluation, planning meetings, and required training.
