Job overview
The Registered Nurse (RN), Per-Visit Home Care Assessments role is onsite for home visits in Baltimore, MD, and surrounding areas. Pay is $45 per hour. Venus Health Care Solutions is a licensed Maryland Residential Service Agency serving Medicaid clients through waiver, Community First Choice, and private-duty nursing programs. The RN completes in-home assessments and supervisory visits, develops nursing plans of care, and delegates tasks to caregivers. The work supports client care through documented assessments, care planning, and caregiver competency verification.
What you'll do
- Complete in-home admission assessments, reassessments, delegation visits, and supervisory visits
- develop and sign nursing plans of care
- prepare CMS-485 forms and review medication administration records
- delegate nursing tasks to caregivers and verify competency
- and submit signed documentation within 48 hours of each visit.
