What you'll do

• Provide direct care and daily support
• assist with personal care, hygiene, meals, medication reminders, and household tasks as applicable
• support communication, social skills, routines, and community activities
• follow individualized care plans and safety procedures
• document services and report changes
• maintain a safe home environment
• collaborate with managers, nursing staff, families, and other support professionals.
