The nurse delivers skilled nursing care during intermittent visits, evaluates patient needs, initiates and updates plans of care, and documents progress under the direction of the Director of Patient Care Services.

What you'll do

• Conduct initial evaluation visits and regularly reevaluate nursing needs
• meet daily productivity standards
• initiate and revise plans of care
• provide services requiring specialized nursing skills
• initiate preventative and rehabilitative nursing procedures
• prepare timely clinical and progress notes and care-conference summaries.
