What you'll do

• Support communication with RN case managers, social workers, and chaplains in the field
• conduct weekly interdisciplinary team meetings
• orient and educate new staff
• conduct quarterly performance check-ins and ride-along visits
• maintain a small caseload
• assess patients and families for hospice needs
• participate in on-call coverage.
