Community Health Worker (CHW)

The Community Health Worker (CHW) role is a hybrid position, with 50% office-based and 50% field-based work in Frederick, MD and surrounding Frederick County communities. The stated salary range is $45,000–$50,000 per year, depending on experience. The Asian American Center of Frederick works to empower immigrants and provides culturally responsive health and social services. The CHW connects community members with health care and other resources through outreach, education, screening, needs assessment, navigation, and follow-up. The role supports access and continuity of care across grant-funded community health programs.

What you'll do

• Conduct outreach and prevention education at community events and partner sites
• run or co-lead wellness circles
• conduct standardized screenings within the CHW scope
• assess care barriers and develop navigation and referral plans
• make warm handoffs to health, social-service, and family-focused resources
• schedule appointments and follow up on referrals
• document contacts and referrals in databases
• escalate participants to intensive coordination when needs increase.
