The physician works with an interdisciplinary team to coordinate care, manage transitions, and support patients’ quality of life.

What you'll do

• Provide primary care to medically complex senior patients
• conduct admission assessments, routine follow-ups, and acute evaluations as appropriate
• develop individualized chronic disease and preventive care plans
• coordinate with nurses, care managers, specialists, social workers, facility staff, and other professionals
• manage transitions after hospitalizations and post-acute admissions
• reconcile medications
• educate patients and families
• maintain timely EMR documentation
• support quality initiatives, risk adjustment, and CMS compliance
• and participate in case conferences and quality improvement.
