What you'll do

• Coordinate implementation of each patient’s plan of care
• identify and reassess patient and family needs at least every 14 days
• provide nursing services and document goals, interventions, and responses
• collaborate with patients, families, physicians, and the interdisciplinary team
• instruct patients and families in self-care when appropriate
• supervise and evaluate assigned hospice aides
• conduct required supervisory visits
• document clinical notes and supervision
• conduct annual aide performance reviews and an annual onsite LPN supervisory visit
• participate in the on-call rotation.
