What you'll do

• Follow each resident’s individualized service plan
• observe and report changes in condition
• document daily care
• check and record vital signs and weights as assigned
• assist with activities of daily living, transfers, and ambulation
• provide prompts and support
• help residents attend activities
• respond to urgent calls
• maintain clean and safe areas
• support fall prevention and a restraint-free environment.
