The Registered Nurse, Home Health role is onsite in Greeley, CO, serving Greeley, Fort Collins, and surrounding areas. Compensation is $43–$52 per hour. Boost Home Healthcare provides home-based care and supports nurses with practice management and clinical resources. What you'll do

• Complete initial and ongoing in-home nursing assessments
• create and update individualized plans of care
• oversee a caseload and adjust services as client needs change
• coordinate with families, physicians, hospitals, and community partners
• maintain accurate records and follow Colorado home care regulations, infection control, and HIPAA guidelines
• provide on-call support for client and caregiver concerns.
