SHP Housing Case Manager

What you'll do

• Advocate and connect participants to behavioral health, medical, and community services
• locate rental units and assist with landlord contact and lease agreements
• support apartment setup and adjustment
• build daily living and self-advocacy skills
• help resolve landlord and neighbor conflicts
• maintain case records and required documentation
• assist with admissions, orientations, entitlements, and discharges
• prepare reports.
