Health Navigator, Street Medicine Team

Job overview

The Health Navigator, Street Medicine Team role is onsite full time in Oakland, CA, with compensation of $24–$25 per hour. The role works alongside the street medicine team in homeless encampments to connect unhoused individuals and families with services. The navigator builds ongoing relationships with participants, assesses barriers to care and stability, and helps connect them with public benefits, medical and social services, and health coaching while supporting clinic operations.

What you'll do

• Build rapport and trust with participants
• conduct face-to-face visits and outreach calls based on participant needs
• assess psychosocial, health, housing, and other barriers
• assist with enrollment in public benefit programs and medical and social services
• provide health coaching such as glucometer training
• support providers and clinical staff
• complete required documentation
• help maintain a clean and safe site
• participate in meetings, supervision, and training.

Why this role

Substance use disorder counseling experience is required. A California driver license and safe driving record are required; two years of nonprofit or community clinic experience and personal or family systems involvement are preferred.
