Registered Nurse Case Manager — Weekend Schedule

The Registered Nurse (RN) Case Manager works onsite in Rochester, NY, on a straight weekend schedule of 32 hours and is paid for 40 hours. Compensation is $33.65–$46.88 per hour. The role supports HCR Home Care’s skilled nursing services for patients at home. The case manager provides and coordinates skilled nursing care under physician orders, oversees each patient’s plan and outcomes, and directs and supervises other nursing staff. The role also includes patient admissions and assessments, family and team teaching, and timely, accurate documentation.

What you'll do

• Provide and coordinate patient care
• maintain a caseload, productivity goals, and accurate documentation
• manage care efficiently to support positive outcomes
• oversee the plan and outcome of care, including directing and supervising LPNs and HHAs
• conduct admissions, obtain medical histories, perform physical assessments, and evaluate patient, family, and home needs
• teach and supervise families and nursing team members in accordance with physician orders.
