Case Manager - Mental Health Pathways

The Case Manager - Mental Health Pathways role is remote, based at Rosecrance Kankakee in Kankakee, IL, with a Monday–Friday schedule from 8:30 a.m. to 5:00 p.m. Compensation is $54,756 per year starting pay for a bachelor’s degree; professional certification may add $0.72 per hour. Rosecrance provides behavioral health services focused on hope and recovery. The role provides mental health rehabilitation and case management to help adults and families reduce hospitalization and crisis episodes and strengthen community functioning. It supports clients’ recovery goals through counseling, education, coordination with community resources, timely documentation, and participation in the clinical team.

What you'll do

• Provide primary case management for designated clients
• conduct individual, group, and family counseling and education
• maintain compliant client records and timely documentation
• coordinate community resources
• communicate with staff about admissions, treatment processes, and discharge plans
• use evidence-based practices and service modalities
• present workshops and training as needed
• participate in clinical team meetings
• maintain confidentiality.
