• Assess patients’ physical, functional, psychosocial, and environmental needs
• implement and revise individualized plans of care
• educate patients and families on disease processes, self-care, end-of-life care, and ethical concerns
• supervise paraprofessionals
• prepare clinical and progress notes
• participate in interdisciplinary group conferences and performance improvement
• maintain client and family satisfaction
• communicate using organizational processes and technology.
