What you'll do

• Conduct Start of Care admissions using OASIS
• complete follow-up and evaluation visits as needed
• visit patients in their homes
• reconcile medications and provide wound care, treatments, and procedures
• monitor patient conditions and report changes to the clinical team
• collaborate with physicians and agency staff
• administer IV medication infusions according to established protocols
• and document observations, interventions, and evaluations. The schedule is every Saturday and Sunday for full days, with consideration for adding three weekdays.
