What you'll do

• Provide person-centered goal setting, education, and support
• collaborate with care managers, supervisors, directors, healthcare professionals, and social service staff
• identify barriers and community needs related to health education and healthcare access
• conduct home or phone visits
• provide informal counseling, social support, referrals, advocacy, outreach, and follow-up
• document services and enter data
• participate in case conferences and team coordination
• maintain confidentiality under HIPAA.

Why this role
