What you'll do

• Evaluate referred members’ needs and eligibility using care management tools and available benefit and community resources
• identify high-risk factors and service needs
• coordinate and monitor care plan activities
• consult with case managers, supervisors, Medical Directors, and other programs
• present cases for multidisciplinary review
• identify and escalate quality-of-care issues
• use negotiation and motivational interviewing to support member engagement
• document care in accordance with regulatory, accreditation, and company requirements.
