What you'll do

• Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans
• Provide counseling and emotional support to residents and their families
• Serve as a strong advocate for residents' rights and preferences
• Connect residents and their families with community resources, support services, and advocacy organizations
• Provide crisis intervention and support during challenging situations.
