The Registered Nurse (RN), Home Health role is onsite in East Kingston, NH, and pays $40 per hour. The nurse provides one-to-one home care, assesses and documents patient progress, coordinates with physicians and care staff, and educates the patient and family to support the plan of care and patient outcomes.

What you'll do

• Provide in-home care for patients with illnesses, injuries, or chronic diseases
• assess patients and document progress
• report changes to physicians
• educate patients on care plans, medication, therapy, diet, and exercise
• coach family members on care and home safety
• coordinate care goals and discharge from services
• support the client with activities of daily living, medication management, and G-tube maintenance.
