What you'll do

• Conduct nutrition assessments for patients with obesity and chronic conditions
• develop personalized nutrition care plans
• provide medical nutrition therapy and ongoing counseling
• support behavior change and lifestyle modification
• collaborate with nurse practitioners, fitness trainers, and behavioral health specialists
• monitor and document progress
• educate patients on evidence-based nutrition
• and support adherence through motivational interviewing and coaching.
