What you'll do

• Assess patients’ medical, psychological, and social needs
• develop individualized care plans with providers and patients
• coordinate treatments, therapies, and consultations
• monitor progress and adjust plans
• educate and support patients and families
• advocate for patient needs and rights
• communicate with providers, insurers, and other stakeholders
• evaluate care-plan effectiveness
• document assessments, plans, and interactions
• participate in a weekly on-call rotation every four weeks for 24-hour work-related injury, illness, or exposure reporting.
