Nursing Care Coordinator (Licensed Practical Nurse or Registered Nurse)

• Review patient records and Review Visit Logs for clinical accuracy and compliance
• identify documentation discrepancies and submit corrections or resolution suggestions
• coordinate with HR, Operations, and Clinical teams
• participate in required meetings and training
• maintain proficiency with listed digital platforms
• review charts for completeness, regulatory compliance, and quality assurance.
