What you'll do

• Maintain contact with children according to service plans
• assess and support emotional, physical, educational, cultural, and social needs
• monitor placement, school, and community progress
• coordinate internal and external services
• attend court hearings and administrative reviews as required
• communicate with families and support progress toward reunification or permanency
• facilitate family visitation and aftercare
• support foster and relative caregivers
• maintain case documentation
• prepare reports and provide court testimony when required
• collaborate with providers and participate in team meetings and supervision.
