RN Case Manager - Home Health

Job overview

RN Case Manager - Home Health, onsite, Peoria, IL. Compensation: $35.87 - $58.37 per hour. $7,500 Sign on Bonus and $2,500 Relocation Bonus available. This role provides professional nursing care to assigned clients, supporting physician objectives and Home Health Services Department policies.

What you'll do

• Provides quality care for clients of all ages
• Conducts client assessments and completes documentation
• Develops, implements, and evaluates client's Plan of Care
• Implements nursing interventions
• Communicates changes in client condition to physician
• Collaborates with physician, client/caregiver, and other health care professionals
• Communicates client/caregiver needs to other staff
• Participates in client case reports
• Reports to assigned Home Health Clinical Manager
• Uses time, supplies, and resources productively
• Observes and performs tests/procedures
• Demonstrates skilled care on an intermittent basis
• Recognizes need for referrals
• Sets goals with client/caregiver
• Provides coordination of care
• Supervises and directs Home Care Aides
• Assesses progression of goals
• Possesses knowledge and skills for lab specimens
• Performs patient bedside lab testing
• Ensures clinical records are complete, legible, accurate, and timely
• Submits clinical documentation per policy
• Demonstrates punctuality and flexibility
• Utilizes time for maximized client care
• Organizes visit schedule
• Recognizes and prioritizes client needs
• Shows flexibility in work schedule
• Attends and participates in departmental meetings
• Responsible for on-call
• Responds to requests for covering extra visits
• Turns in cellular phone statements
• Submits payroll time slips
• Obtains and utilizes educational opportunities
• Assumes responsibility for being informed
• Informs and interprets health information
• Identifies own learning needs
• Meets and maintains all educational requirements
• Participates in work groups/teams
• Knowledgeable of payor sources requirements, OASIS Data Sets, Standards of Professional practice, HHSD policies and procedures and regulatory requirements
• Obtains/maintains technical skills, competencies or certifications.
