What you'll do

• Assess patients and families for hospice needs

• use assessment data to guide care plans
• coordinate care with the healthcare team, patient, and family
• counsel patients and families on end-of-life needs
• initiate preventative and palliative nursing procedures
• communicate patient needs and condition changes to physicians
• supervise hospice aides.
