What you'll do

• Assess new admissions and determine health status and level of care
• complete and track MDS assessments, including Medicare, quarterly, annual, and significant-change assessments
• coordinate interdisciplinary participation
• maintain assessment schedules and reference dates
• enter and submit data electronically
• conduct resident care conferences
• assist with care plans
• participate in restraint reduction and fall management
• coordinate with therapists.
