What you'll do

• Complete initial psychosocial and bereavement risk assessments within five calendar days of hospice election
• obtain and follow up on community-resource referrals
• assist with facility placement when appropriate
• provide psychosocial and anticipatory grief counseling
• assess coping and communicate findings to the team
• participate in care planning and case conferences
• prepare social histories, evaluations, and interventions
• maintain clinical documentation and assist with discharge planning
• attend interdisciplinary team meetings
• support patient and family education
• mentor and assist with orientation and field training.
