What you'll do

• Assist with shopping, errands, hygiene, and medication administration
• support mobility, including two-person transfers and Hoyer lift use
• help clients with cooking, cleaning, and social integration goals
• transport clients to appointments and community events
• document progress on Person Centered Service Plans
• communicate with supervisors and team members.
