Resident Care Coordinator

What you'll do

• Schedule and assign caregiver and medication technician shifts
• participate in recruiting and interviewing
• supervise, train, and support care team members
• oversee the medication room and orders
• maintain resident records and care plans
• coordinate care planning with home health agencies
• coordinate admissions and move-ins
• conduct admission interviews
• communicate with residents, families, and providers
• address resident needs and concerns
• assist with medication administration and direct care as needed.
