What you'll do

• Assess patient and caregiver needs
• develop and communicate individualized plans of care
• manage assigned cases and visit schedules
• implement medical plans, nursing interventions, and procedures
• maintain high-tech skills including IV use and complex wound care
• collaborate with LPNs
• review and document care
• evaluate patient responses
• educate patients and caregivers.
