The role provides direct services, prevention education, testing, outreach, and referrals while coordinating client care and documenting progress. It also supports food distribution and educational workshops to help clients access resources and coordinated services.

What you'll do

• Provide direct services and prevention education
• manage a caseload
• administer HIV and Hepatitis C testing
• connect clients with program resources and medical providers
• facilitate evidence-based interventions
• provide PrEP and PEP education and referrals
• coordinate with social service, mental health, substance use, and medical providers
• conduct outreach
• complete intake, risk assessment, service plans, referrals, advocacy, and counseling
• document progress and prepare reports
• receive, sort, store, and distribute food
• drive the agency medical van to testing and outreach locations.
