Hospice Nursing Assistant, STNA/CNA

The Hospice Nursing Assistant role is onsite in Sandusky, OH, with a 40-hour workweek and every-other-weekend rotation; staff also work every other holiday on a rotating schedule. Compensation is not specified. Reserve Care provides hospice, palliative, and grief support, with a focus on comfort, quality of life, choice, and support for patients and families. Under nurse supervision, the assistant provides personal care according to each patient’s plan of care, supports safe mobility and comfort, records care and observations, and communicates changes to the nurse and care team.

What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• assist with ambulation, transfers, and range-of-motion exercises as directed
• prepare nutritional snacks as appropriate
• assist with certain self-administered medications in home care under nurse supervision
• perform simple wound, skin, and bowel care protocols as directed
• report changes in patient status
• document care and observations
• attend care conferences and required meetings and in-services
• participate in scheduled on-call rotation for home care only.
