Family Care Coordinator, Case Manager, Social Worker

The Family Care Coordinator role is onsite in Hazard, KY, with a flexible schedule that may include nights and weekends, regional travel, and participation in an on-call rotation. Compensation is not specified. KVC Kentucky provides in-home behavioral healthcare, family preservation and reunification, foster care, and youth substance-abuse treatment to children and families. The coordinator works directly with families to assess safety, connect them with resources, and coordinate services toward treatment goals. The role supports child well-being and family preservation by maintaining communication among families, providers, and DCBS personnel and helping remove barriers to care.

What you'll do

• Contact families within 24 hours of referral to assess safety, initiate safety planning, and connect them with immediate resources
• conduct team meetings and monitor progress toward treatment goals
• coordinate providers and services under each family’s Action Plan
• maintain frequent contact with DCBS personnel
• provide direct services to reduce risks, improve child well-being, and preserve child placement with the family
• travel throughout the region and participate in an on-call rotation.
