What you'll do

• Provide care to seniors and people with illness, injury, or chronic disease in their homes
• assist with bathing, toileting, dressing, grooming, hygiene, nail and skin care, eating, and nutrition
• assist with ambulation, transfers, and range-of-motion exercises
• check vital signs, measure intake and output, monitor blood glucose, and document changes
• follow the patient’s care plan with home health nurses and aides
• notify a supervisor of issues requiring changes to care, medication, or equipment.
