What you'll do

• Conduct behavioral health assessments
• develop, implement, and monitor individualized care plans with clients and healthcare teams
• coordinate referrals to community resources, treatment providers, and support systems
• provide case management, crisis intervention, and follow-up
• document progress and maintain accurate records
• educate clients and families about conditions, treatment options, and coping strategies
• collaborate with multidisciplinary teams to advocate for client needs.

Why this role

Requires a bachelor’s degree in social work, psychology, counseling, or a related behavioral health field and at least two years of behavioral health case management or consulting experience. A master’s degree, LCSW/LPC or equivalent licensure, integrated-care experience, and training in trauma-informed or evidence-based interventions are preferred.
